- Payment will be deducted from your Credit Card or Paypal when you check out.
What's Included:
Purchasing this admin service means that we will:
- Email you the parent request questionnaire and let you know what else we need from you, we will also let you know the date you can expect to receive your completed work by.
- Once we get your questionnaire and any relevant documents we will then use all the information to complete our Parent Request template.
- We will then email this back to you to send off to your LA to start the EHCP Process.
What Happens Next?
After you enter your payment details, you'll be taken straight to your library and you'll get an email with instructions.
